Factors Affecting the Response to Gemfibrozil in Hypertriglyceridemic Continuous Ambulatory Peritoneal Dialysis(CAPD) Patients

Abstract
The CAPD Patients have a high incidence of hyperlipidemia, especially hypertriglyceridemia. Gemfibrozil, a fibric acid derivatives, was considered to be effective in the treatment of hypertriglyceridemia, But individual variations on the response
to
gemfibrozil treatment were noted. This study was performed to assess the factors affecting the response to gemfibrozil on hypertriglyceridemic CAPD patients. The subjects were recieving CAPD for more than 3 months with serum cholesterol levels
above 250
mg/dl and/or serum triglyceride levels above of 250 mg/dl. The possible factors analyzed were sex, age, diabetes mellitus, use of beta blocker, physical activity, smoking, percent ideal body weight, duration of CAPD before therapy of gemfibrozil,
serum
albumin, creatinine, triglyceride level before therapy.
@ES The results obtained were as follows:
@EN 1) The patients were 13 men and 9 women, mean age 48.1 years (range 28 to 68 years). The mean duration of CAPD before therapy of gemfibrozil was 14.9 months (range 3 to 48.7 months).
2) The serum triglyceride levels decreased from 428.7¡¾216.6 mg/dl to 258.2¡¾118.7mg/dl and the percent change of triglyceride was -35.6¡¾19.9% after 12 months of gemfibrozil treatment.
3) The serum total cholesterol levels decreased from 241.9¡¾63.6mg/dl 231.6¡¾48.8 mg/dl and the percent change of cholesterol was -1.2¡¾18.7% after 12 months of gemfibrozil treatment.
4) The serum HDL cholesterol levels decreased from 26.4¡¾7.4 mg/dl to 32.8¡¾6.7 mg/dl and the percent change of HDL cholesterol was +27.5¡¾28.8% after 12 months of gemfibrozil treatment.
5) The univariate analysis showed that the significant factors on the response of gemfibrozil were sex and status of physical activity. The percent decrement of triglyceride was significantly higher in male patients with -43.0¡¾17.3% than female
with
-25.0¡¾19.5%(p<0.05) and patients with active physical status responded better than inactive physical status with -42.3¡¾16.4% and -24.0¡¾21.2, respectively(p<0.05)
6) The multiple regression analysis showed that serum creatinine and triglyceride levels before therapy of gemfibrozil were significant factors affecting the response of gemfibrozil (significant T value < 0.01).
In conclusion, the predictive factors significantly affecting the response of hypertrigly ceridemic CAPD patients to gemfibrozil were suppose to be serum creatinine initial, before therapy triglyceride leved. Sex. Status of physical activity.
